I've had it explained that MH patients don't get their summaries because what they read might worsen their condition by upsetting them. Organisations can also reject applications for Freedom of Information to view their files if they feel the patient is too unwell to cope with reading them.
Some hospitals are changing such policies on discharge summaries though.
